Though I have to say, I think you missed an opportunity for some extra potential by having him actually orgasm at the moment of death.
If he'd died JUST before finishing, ruined orgasm etc. With no orgasm for weeks before (because weak heart and lack of opportunity.)
Then there'd be that whole 'spirit with unfinished business' trope to call on.
Also the 'spirit reliving their last acts' stuff, so when as a ghost he reaches orgasm,it always instantly ruins.

Then there's an obvious final resolution theme too. After many struggles he is able to move on, taking the tunnel of light. Except he's finally orgasming, stroking triumphantly.
