On the one hand it's entirely possible that none of his peers knew he was in the film. I'm sure he wouldn't exactly be broadcasting his role in this film but while there's simple ways around it, children wouldn't have been technically allowed to view it nor would children have been lining up to see this film. In fact, hardly anyone went to see it as it was a box office bomb. Even Mark Dightam was technically not allowed to view the film when it was released because he was under 15.

But imagine if one of them did. Imagine as he entered middle school one of the girls told him she saw him in the film, and he had to look her in the eye if he was even able to after hearing that and hope she wouldn't be malicious and tell other kids. Just by saying 'I saw you in Macbeth' says so much more than the specific words of that sentence. She would be saying she saw everything at a very sensitive time in his life. It may never have happened but the fact that even the possibility was set up feels wrong.

Also, high schools all over the world still show this specific version all the time. Assuming the film was watched from beginning to end, there's no telling how many people have seen this boy completely naked. The thing I still can't get past is the specific director being who he is. I have to question his intentions with this 'artistic' choice he made given 5 or 6 years later he was arrested for sexual abuse of a minor.

The actor later said doing this scene did not traumatize him. He was actually comfortable with it. He said he walked about the set
completely naked between takes. The actor said he was fourteen, and was chosen due to looking much younger.
The scene did cause some controversy when the movie came out. This was back in 1966 when few people had seen what movies were to
become during the end of the 60s through the 70's for casual nudity.

This scene would be shocking if filmed today, especially when the mom tells the boy to jerk off out in the open, that it would be
preferable to filthy secret jerking off.

It probably was not seen by that many people. It is possible kids as school did not recognize him as the actor in this movie.

You’re probably right that not many of the kids he knew saw the movie at the time. I’ve always just wondered what it’s like for people who have appeared nude in a movie after it’s come out.
