I know my answer to this is always "context!" and my answer is still that it depends on the context.

I think most of the time it would be genitals, especially if she's making me "prove" I'm a boy or there's about to be a discussion of my size or state of erection.

But if this is a butthole inspection and she's checking for hygiene/cleanliness or evidence of recent sexual activity, that would be worse. Especially since pretty much any position that would reveal my anus is going to display the back of my scrotum as a "bonus".
