This is not from a movie but from a TV show. It was called "Vitamina N" and aired in Catalonia in the early 00s, maybe 2003-04. It was a late show and included a contest where the participants had to strip and the loser ended naked. In this case the participants were two girls and well... they tied
Skip to 8:25 for the naked part, and at 11:15 the girls go outside to the streets for a challenge (asking people for money).
I used to watch it, men also participated in this strip contest and ended naked, not only females. Sadly the show didn't last long and was cancelled shortly after

my guess from the way its shot is he didnt actually see her naked like that. would be so cool for him tho if he did
Given when this was shot, I think there's a good chance they didn't bother putting that much effort into it and simply had him be there and see her
Usually the tells are pretty obvious -- eyelines don't match, reaction doesn't seem genuine, the two actors never appear in the frame together, and especially when the shot rhythm changes and the shot compositions isolate one or both characters and it feels like the shots could even have been taken on different days.
In this case though... With the set design it doesn't loan itself to a two-shot to begin with. The focus of the moment is on the brother's non-reaction so from a film grammar perspective it fits. His non-reaction is odd but seems genuine and the eyelines might be a little off but not egregiously so and might be more of a function of his body posture and camera angle. Importantly, the lighting seems to match.
Given how much nudity and semi-nudity there is throughout the film, I'm certain she's there on set for the shot. And since the two of them share at least one bathtub scene, it's at least possible she could have remained naked. But my guess is she's probably back in bra and panties. There wouldn't really be a reason for her to stay exposed on the set since the scene ends right after. Plus her very white underwear would have been just as effective for him to focus on.

Thanks for explaining the story, but I think it was clear already the girl in the bathtub was a different one. I wrote "he definitely saw his OTHER sister".
Being a Stargate fan, I can't believe I didn't think about this one earlier. It's from Stargate SG-1's pilot episode "Children of the Gods". A woman is chosen to be the next host of a goa'uld, a parasitic snake-like creature, and is stripped naked. The actress is the gorgeous Vaitiare Bandera.
This is from the film "Les Diables". No link in English but I'll explain the story. A sister and brother are left at a sort of home for children with no one to care from them. The 12-year-old sister is autistic and mute, and the brother tries to take care of her, even when they're separated by gender in the home. The actress, Adele Haenel, was 12 or 13 at the time of the filming and she had a good career before renouncing acting. (She denounced the French film industry for giving an award to Roman Polanski.). She's also openly lesbian and had a long relationship with the director Céline Sciamma; friendly split and they worked together in her last film, "Woman in Flames." (Lesbian love story, also with Noémie Merlant who you might remember from "Tar" - both appear nude, in case anyone is interested!)

Wondering if anyone knows anything about Jambon d'Ardenne (1977, lit. "Ham of the Ardennes", apparently translated to Ham and Chips). There's a scene starting around 14:40 of swimming in the pond that suddenly becomes a skinny dipping scene, followed immediately by a scene of a boy and girl frolicking in the grass. The camera acts coy for few moments just showing us the girl's upper half before dropping the pretense and giving us a few glimpses of full frontal.
